Graduate Student Forms
**Due to the COVID-19 pandemic, all thesis defense forms and committee meeting forms are only available online, we will return to normal business practices once we return to MSU**
Effective September 9, 2020 all graduate students must follow the new procedures that were sent via email (and listed below) and use the most recent version of the committee meeting forms that are also posted below.
First guidance committee meeting report - updated 11/20/2020

Upon completion of any guidance committee meeting (1st - Nth) all forms must be saved in 1 PDF along with approvals from all committee members (either digitally signed or email form) and sent to graduate program director, who will review, sign and forward to PA graduate secretary for
processing and recording in GradPlan of RCR training.
It will remain the responsibility of the graduate student to monitor the status of GradPlan and follow up with their committee members as needed. Documenting of RCR training is not fully complete until everyone on committee has approved.
PhD Completion Form -
Upon successful completion of PhD Thesis Defense, form must be saved in 1 PDF along with approvals from all committee members (either digitally signed or email form) and sent to PA graduate secretary for processing and recording in GradPlan.
